Acknowledgement of Process
The Office of Student Rights and Responsibilities administers the Campus Student Code and is tasked with
investigating allegations of prohibited conduct.
Advisor Form
The OSRR will provide this form to all students prior to their disciplinary hearings. If the student chooses to have an advisor, this form should be filled out and provided to the Office of Student Rights and Responsibilities. All advisors must be a WVU faculty, staff, or student except in cases involving BOG Rule 1.6 violations.
Attorney Form
The OSRR will provide this form to all students prior to their disciplinary hearings. If the student chooses to retain an attorney, this form should be filled out and provided to the Office of Student Rights and Responsibilities. All attorneys serving in this capacity must be admitted to practice law in the state of West Virginia.
Authorization to Obtain Information
For students who wish to authorize the OSRR to receive information from the Carruth Center for Counseling and Psychological Services.
Consent to Disclose / FERPA
In order to obtain records that contain personally identifiable information about a student who is or was enrolled in an educational institution, the Family Educational Rights and Privacy Act (FERPA) requires that the party receiving the information agree not to disclose that information to any other party without the prior consent of the personally identified students.

Hearing Advisor Request
The Office of Student Rights and Responsibilities will provide this form to all students who do have an attorney but desire legal advice for disciplinary hearings.
